The Tunnel Hill 50 Mile follows the Tunnel Hill State Trail out of Vienna, Illinois, running out-and-back to a southern turnaround at the Cache River State Natural Area's Wetlands Center and a northern turnaround beyond the community of Tunnel Hill, crossing the trail's namesake railroad tunnel twice. The route holds a grade under 2 percent over its converted rail-line profile. USATF and World Athletics certified, it has produced multiple 50-mile world records, including Anne Flower's 5:18:57 in 2025 and Charlie Lawrence's 4:48:21 in 2023.
